Transcript Services

To request an official transcript, please complete the Transcript Request Form and mail it along with a $7.00 processing fee (check or money order) to:

Transcripts are generally processed within 21 business days from the date of receipt.

Enrollment Verification

For verification of past enrollment, email transcripts@aii.edu with the subject line:
Enrollment Verification Needed

Include the following in your message:

  • Full name (First, Middle, Last)
  • Mailing address and phone number
  • Campus attended and dates of attendance
  • Program of study
  • Any applicable verification forms as attachments

Diploma Replacement

To request a replacement diploma, contact studentresolution@dcedh.org with the subject:
Diploma Request

In your email, please provide:

  • Full name as it should appear on the diploma
  • Current mailing address
  • Campus and program completed
  • Degree level and honors (if any)
  • Completion date and attendance dates

Financial Assistance and Records

For help with financial records or tuition matters, email studentresolution@dcedh.org using the subject:

Your message should include:

  • Full name used during enrollment
  • Name of the campus attended
  • Detailed explanation of your financial inquiry

Tax Forms – 1098-T

Annual tax forms are distributed by January 31. If you need a duplicate or further assistance, visit: https://heartland.ecsi.net or contact Heartland ECSI at 1-866-428-1098.
